Pregnancy Administrative Procedures

Obtain the Pregnancy Notification leaflet that states your expected due date from your OB/GYN.
Install the Boshimo App on your device.
Use the app to submit the Pregnancy Notification and to apply for the Maternal and Child Health Handbook.
You can also use the same form to apply for the first Maternity Support Benefit.
Request for a Maternal and Child Health Handbook.
Pick up your handbook at a Public Health and Welfare Center.

Expectant Mother's My Number Card
Bank Book or Cash Card that states the name of the financial institution, account number, and account holder's name
